Patients can make repeat prescription requests using 3 methods:
- Local pharmacy request
- Website prescription request form
- Patient Access application
All medication requests will be actioned by the GP within 2 working days
- Local Pharmacy Request:
Any patient on repeat medication with a nominated pharmacy can contact their pharmacy to request their medication on the patient’s behalf.
2. Website Prescription Request Form:
Any registered patient can use our prescription request form on the practice’s site: https://fortunegreengp.com/prescription-request-form/.
This form asks patients for their name, date of birth, address, contact details, medication required and nominated pharmacy. Patients can also add a message for the GP if it is required.
3. Patient Access application
Patients who have access to Patient Access can request repeat medication using the application.

4. Handwritten requests
Some patients may feel more comfortable writing their medication requests and handing it to our reception staff. The practice accomodates these requests as long as the full name, date of birth and medication requested is recorded clearly.
